Patients pay $75 for a 30 day supply of IYUZEH under the cash program

Privacy/Attestation

In order to participate in the IYUZEH™ Copay or Cash Program and receive this benefit, patients must meet certain eligibility criteria. During the enrollment process, patients will be asked to provide personal information that may include name, address, phone number, email address, date of birth, and information related to insurance and treatment. This information is necessary to determine eligibility and participate in these programs.

By enrolling in the IYUZEH™ Copay or Cash Program, patients permit Thea Pharma Inc. and its vendors and affiliates, to provide benefits related to the use of the IYUZEH™ Copay or Cash Program cards (digital or print), if applicable. If a patient enrolls in either of the above IYUZEH patient access programs, it serves as an agreement to allow Thea Pharma Inc. or its agents to contact the patient about program information via email or mail in the future.
